List of SSS Member / Membership Requirements for Employees

SSS MEMBER REQUIREMENTS – Here is a list of the requirements that employees have to prepare in applying for Social Security System (SSS) membership.

Whether you are a non-working individual or an employee of a private company or sector, you may apply for membership to Social Security System or more commonly known as SSS.

The SSS is the social insurance program ran by the government. It offers claims and benefits to its members like loans, monthly pension, and personal benefits.

Are you up to apply for membership to the Social Security System? If you are an employee, you may start with obtaining an E-1 Form and preparing the SSS member requirements.

SSS Member Requirements

To apply for SSS membership as employees, you have to prepare the following SSS member requirements:

  • Baptismal Certificate
  • Driver’s License
  • Professional Regulation Commission (PRC) ID
  • Passport
  • Seaman’s Book

In case you don’t have one of the identification documents stated above, you may replace it with any of the following according to the Social Security System:

  • ATM card with the cardholder’s name
  • ATM card and certification from the issuing bank that the account number belongs to the cardholder, if the card does not bear the cardholder’s name
  • Bank Account Passbook
  • Birth/Baptismal Certificate of children
  • Certificate from: Office of the Southern/Northern Cultural Communities or Office of Muslim Affairs
  • Certificate of Licensure/Qualification Document/Seafarer’s ID & Record Book from the Maritime Industry Authority
  • Certificate of Naturalization issued by the Bureau of Immigration
  • Company ID
  • Company Representative Authorization Card issued by SSS
  • Credit Card
  • Fisherman’s Card issued by the Bureau of Fisheries and Aquatic Resources (BFAR)
  • GSIS Card/Certificate of Membership
  • Health or Medical CardID card issued by Local Government Units (e.g. Barangay/Municipality/City)
  • ID card issued by professional organizations recognized by the Professional Regulation Commission (PRC)
  • Life Insurance Policy
  • Marriage contract
  • Membership card issued by private companies
  • NBI Clearance
  • Overseas Workers Welfare Administration (OWWA) card
  • Pag-Ibig Member’s Data Form
  • PHIC Member’s Data Record
  • Police Clearance
  • Postal ID card
  • School ID
  • Seafarer’s Registration Certificate issued by the Philippine Overseas Employment Administration (POEA)
  • Senior Citizen Card
  • Tax Identification Number (TIN) Card
  • Temporary license and Student Permit issued by the Land Transportation Office (LTO)
  • Transcript of School Records
  • Voter’s Identification Card/Affidavit

Submit the duly-accomplished E-01 Form together with the requirements. It will be checked by an SSS personnel and if there is no problem with it, you may proceed to your application depending on the flow observed by the SSS office.
