For several days, Tito was unconscious in the hospital after he tripped on a cable wire while taping for Kapuso teleserye Rosang Agimat.
On June 20, he passed away and the entertainment industry lost a legendary actor and director.

Based on the report from Bombo Radyo, the Occupational Safety and Health Center (OSHC) under DOLE is now investigating the incident.
DOLE Secretary Silvestre Bello III said that OSHC started to investigate if GMA 7 really committed negligence on this matter.
The investigation will also reveal if the TV network is implementing the safety and health plan.
