- Personal Appearance of a
minor applicant and either parent or authorized adult companion - Philippine Statistics Authority (PSA) Birth Certificate
- Marriage Certificate is required if only one parent is accompanying the child
- Passport or Valid Government issued ID of either parent
- School ID (if applicable)
For Adult Applicants (18 Years Old and Above)
- Confirmed Online Appointment
- Personal Appearance
- Accomplished Application Form
- Original copy of Philippine Statistics Authority (PSA) Authenticated Birth Certificate on Security Paper
- One valid ID and photocopy
