SSS DEATH BENEFIT REQUIREMENTS – Here is a list of the requirements that beneficiaries should prepare in applying for a Social Security System (SSS) Death Claim.

One of the benefits that Social Security System (SSS) members are entitled to is a death claim. It is given to primary beneficiaries of the deceased member or the secondary beneficiaries in the absence of the former.

For SSS members who have paid not less than 36 monthly contributions, their beneficiaries may be granted the claim through a monthly pension.

For SSS members who have not reached 36 monthly contributions, their primary beneficiaries may avail the claim through a lump sum amount.

SSS Death Benefit Requirements

There are SSS Death Benefit requirements that beneficiaries should prepare in applying for the claim. Based on the SSS’ official website, beneficiaries should comply with the following:

  • Death Claim Application (SSS Form DDR-1);
  • Affidavit of Death Benefit, if a claimant is a secondary beneficary (SSS Form CLD-1.3A)
  • Filer’s Affidavit (Sinumpaang Sanaysay)
  • Other Affidavit, whichever is applicable
    • Joint Affidavit of Two Disinterested Persons, if the claimant is legal heir or designated beneficiary (SSS Form CLD-1.3)
    • Application for Appointment as Representative Payeee, if claimant is guardian (SSS Form CLD-15)
  • Report of Death (SSS Form BPN-105), if death is work-related
  • Claimant’s photo, signature form and valid IDs
  • If the claimant is a spouse of the deceased, marriage certificate and birth certificates of minor children (duly certified by LCR/NSO)
  • If single, the deceased member’s birth certificate and marriage certificate of parents (duly certified by LCR/NSO)
  • Certified true copy of deceased member’s death certificate
    • certified/issued by LCR/NSO, if the member died in the Philippines
    • issued by vital statistics/census office or equivalent agency and certified by the Philippine Embassy/Consultant, if the member died abroad
  • For pension – Single savings account passbook or ATM card with validated deposit slip or Cash Card Enrollment Form (photocopy and presentation of original for validation)

According to the Social Security System, the office may request for other documents aside from the SSS Death Benefit requirements stated above.
