List of Requirements In Applying for Philippine National ID Card / PhilSys ID Card
PHILSYS ID CARD – Here is a list of the requirements in applying for a Philippine National Identification (ID) card.
Previously, the executive order that seeks to provide a Philippine national ID card to the Filipino citizens was approved. It mandates the creation of the Unified Multi-Purpose Identification Card or UMID ID.
The said identification card is a document that can be used in transactions to several government agencies in the country including:
- Government Service Insurance System (GSIS)
- Social Security System (SSS)
- Pag-IBIG Fund / Home Development Mutual Fund
- Philippine Health Insurance Corporation (PhilHealth)
Previously, it was also suggested that the UMID Card or PhilSys ID would be used as a voter’s ID in the country. Here is a list of the information that can be seen in the ID card:
- Complete Name (Given Name, Middle Name, Surname)
- Gender
- Date of Birth
- Place of Birth
- Address
- Citizenship
- Marital Status
- Mobile Number
- E-Mail Address
In applying for the PhilSys ID, there is a list of requirements that applicants should prepare. Based on an article on Pinoy Moneytalk, here are the documents needed:
- Duly-Accomplished PhilSys Application Form
- PSA Birth Certificate
- One(1) Government-Issued ID bearing the name, photo, and signature of the holder/applicant
According to the article, not only Filipinos but as well as foreigners residing in the Philippines may apply for the national identification document. A foreigner should present an ACR ID in applying.
The full implementation of the PhilSys ID has yet to start. A trial run was done by the Philippine Statistics Authority (PSA) before and its full implementation would depend on the result.
In case it would be implemented fully which is expected to happen by September 2019, applicants may apply for the PhilSys ID through any of the following:

- Postal office (under PHL Post or Philippine Postal Corporation)
