SSS UMID CARD – Here are the steps on how members of the Social Security System (SSS) can apply for the Unified Multi-Purpose ID or UMID card.
One of the beneficial valid IDs that you can have is the Unified Multi-Purpose ID or more commonly called UMID card. It is a combined ID of the following government agencies:
- Social Security System (SSS)
- Government Service Insurance System (GSIS)
- Philippine Health Insurance Corporation (PhilHealth)
- Pag-IBIG Fund / Home Development Mutual Fund
Both members of the GSIS and the SSS can apply for the said identification card at the respective social insurance institution they are a member of. Different sets of requirements may be asked.

Step 1
Download the Unified Multi-Purpose ID application form from the official website of the Social Security System (SSS). Provide the information requested on the form.

Step 2
Submit the duly-accomplished UMID card application form and the requirements to the nearest SSS branch office.

Step 3
The SSS staff will check your documents and the next parts would include the taking of the ID photo, fingerprints, and signature.
Step 4
Wait for the releasing of your UMID card. It will be released within eight(8) working days and will be delivered within thirty(30) days since the date of your application.
